INITIAL Visit

Your child’s initial appointment will consist of an examination and review of your child’s treatment options. We usually clean your child’s teeth during the visit.  We may offer you an appointment to return another day for treatment or periodic cleaning & check-up.

Please assist us by completing the following information prior to your child’s 1st visit:

• Complete all electronic forms via the link you received.
• Send us digital x-rays taken by a previous dentist within the past 6 months.
• Send us a picture of your child’s dental insurance card’s front & back, if applicable.
• Send us guardianship documents, if applicable
• We require a driver license or state ID at the time of the visit.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.
